Please fax all Encore Exit Forms to 619-725-7367 (forms should not be sent to Encore On-Site Support). Forms can also be submitted through school mail to: Ed Center, Annex 2 Attn: Encore Exit Form. Please do not mail AND fax as this creates duplicates.
